Root Cause Analysis for Participants:
1-day
25 maximum
PC 501
Provide the knowledge and skills necessary to effectively participate in problem analysis.
Anyone who might participate in an incident investigation, or the general employee who needs to become a better problem solver, but will not be asked to facilitate an investigation.
This course provides a working knowledge of the Apollo Root Cause Analysis (RCA) methods. Students learn how to overcome the natural tendency to tell stories, and to replace this propensity with causes and evidence. When all players know the simple rules of the Apollo methods, the investigation time is significantly reduced. Students learn not to waste time making excuses or selling their favorite solutions. By learning these basic problem solving skills, they become more proactive. They identify and resolve issues before they become major problems, because they realize things do not just happen. Students learn that problems are caused by actions and conditions, and by controlling the causes, we can prevent unwanted effects. This course includes a 96 page training manual and handy reference cards. Effective learning goes beyond the classroom, so we require each student to complete an Apollo Cause and Effect Chart using one of their own problems. The students are requested to send their report to us within 30 days. We review, comment and return it to the student with an evaluation and a certificate of completion.
