Organizational Structure (Roles and Responsibilities):
We
have seen most successful RCA Programs begin with the creation of a Program
Champion who reports at the highest level in the organization. The program
champion has overall responsibility for implementation and results. The
Program champion is supported by Super-users, who are highly skilled facilitators,
that lead the implementation in their respective departments or sites.
The Super-users are supported by local Facilitators who act as the primary
analysis leaders and are responsible for reporting and action follow up.
Each Facilitator is supported by trained participants who have had at
least one day of Apollo RCA training. While each member of this structure
is important to success, the keystone is management support. Managers
from the top down must be able to recognize an effective RCA and demand
nothing less.
