Incident Investigation Services:

Often times incident investigation is better performed by an outside entity. Apollo can provide investigators who are unbiased and experienced. Our personnel have many years of investigating experience. We can perform the entire investigation to include data gathering, facilitation, cause and effect charting, and writing the formal report. We can also simply provide expert facilitators, which is a common practice for many of our clients.

Third Party Facilitation:

When two parties disagree as to the best approach to take, we offer third party facilitation to help both parties better define the problem and identify an effective solution that both can agree on.

RCA Program Assessment:

The purpose of a gap analysis would be to assess the effectiveness of the RCA program within your organization against RCA Program Institutionalization best practices. The analysis is performed by conducting interviews from a representative sample of the entire organization and auditing documents that provide information about your company’s goals and objectives, metrics and threshold criteria, reporting, tracking, and continuous improvement.
